• Personal Information
  • Date of birth
  • Place of birth
  • Martial status
  • Social Security number/card
  • Medicare card
  • Medicaid card
  • Insurance policy card
• Medical Information
  • Doctor's orders including: medications, treatments, diet orders, activity level.
  • Will the doctor follow them to the nursing home.
  • Current history and physical
  • Recent lab and X-rays
  • Discharge summary from last current hospitalization
• Financial Information (for planning purposes)
  • List of assets, income, and liabilities
  • Legal documents
  • Copies of Durable Power of Attorney for Health Care, Advanced Directives/Living Will, Legal Guardianship.
