  • Is the atmosphere warm and happy?
  • Is the staff courteous, helpful and caring toward the residents?
  • Is the office staff friendly and helpful in showing the facility?
  • Is there a clean smell throughout the building?
  • Do the residents seem content, clean and well groomed?
  • Are the outside grounds well kept?
  • Are there places for you to visit with your loved one other than their room?
  • Is there a copy of the resident's rights available to visitors and residents?
  • Are the call buttons convenient and easy to reach?
  • Is the facility licensed to accept Medicaid?
  • Are activities planned for each day of the week, including weekends?
  • Are there activities for the inactive and bedridden residents?
  • Are the residents offered worship services and other spiritual activities such as Bible studies and hymn sings?
  • What are the staffing levels, do they have enough staff for the number of residents they are caring for?
  • Is the dining room attractive and inviting?
  • Are special dietary needs met?
  • Is there a waiting list?
  • Does the facility accept Alzheimer's residents?
  • How does the home encourage family participation?
